1) Copy of medical permit (embossed copy w/doctor's signature) or both sides of medical permit card. Please enlarge by 200% so it is legible on our side.

2) Color copy of your current ID or Drivers License enlarged by 200%.

Once we have received and verified your information your order will be placed within 48 hours.
